For Parents / Carers:
- Read the Information for Parents and Carers document
- Sign the Student Expresion of Interest form and return to the school along with the Student Needs Assessment form and Parent/Carer Agreement form
- Provide emergency adult contact
- Sign the SBAT contract, if student is under 18
